By executing this document, the undersigned representative of the business hereby represents, warrants, and certifies, under penalty of perjury, that: (i) I am authorized to complete and submit this Credit Application and (ii) the information provided in this Credit Application is truthful, accurate and complete.
The business hereby acknowledges that if JPC Equestrian Inc. extends credit to the business, it is doing so based, in part, upon its reliance on the representations in this credit application.
The business hereby approves of the credit application and consents to JPC obtaining credit reports on it and contacting the Bank references and Trade References listed herein.
